Treatment of Subtrochanteric Femoral Fractures with Locking Compression Plate : Comparative Study with Dynamic Hip Screw

Abstract
Purpose: The aim of this study is to evaluate the efficacy of distal femoral locking compression plate(LCPDF) in subtrochanteric femoral fractures compared to the group which used compression hip screws.

Materials and Methods: Among patients who received internal fixation for the femoral subtrochanteric fractures from December 2002 to June 2007 except pathologic fractures, we analyzed 29 patients who were followed up over 1 year. They were divided into 2 groups, 12 cases which were treated by using LCPDF(group 1, 19-91 years) and 17 cases using compression hip screws(group 2, 29-100 years) while the discrepancy was compared and evaluated by analyzing the
time of bone union, functional recovery of hip joints, and complications related to surgery.

Results: An average 15.5 weeks bone union was obtained from Group 1 while an average 16.5 weeks from Group 2. Since the range of motion of the hip joint was recovered almost to its original condition in all cases. Even though there were 1 case of conversion to artificial joint replacement because a failure of the fixation of the proximal area from Group 1 was generated, 1 case of separation of the compression hip screw and plate from Group 2, and 1 case of intra-articular perforation of lag screw; there were no cases of main complications such as delayed union, non-union, infection, or other axial/rotational changes.

Conclusion: Fixation using LCPDF for femoral subtrochanteric fractures was useful without any significant difference compared to fixation using compression hip screws and is thought to be especially effective when there is a fracture in the greater trochanter.
